Rates increase at five year intervals until age 75 when coverage ends. The premium contributions shown reflect the current rate and benefit structure. Premium contributions may be changed by New York Life Insurance Company on any premium due date, but not more than once in any 12-month period, and any date on which benefits are changed. However, your rates may change only if they are changed for all others in the same class of insureds. For example, a class of insureds is a group of people with the same issue age. Benefit option amounts are not guaranteed and are subject to change by agreement between New York Life Insurance Company and the Group Policyholder.
The open enrollment period for this offer is September 15, 2024 through November 15, 2024. After this period, you will only be able to apply for Group Term Life Insurance through the Simplified Issue or Standard Issue process described in the Group Term Life Insurance section of this website.

EXPRESS – up to $250,000 under age 50, or up to $150,000 under age 60. Apply by answering just five medical questions using our simplified form – no physical exams required. (In some cases, additional information may be required.)
STANDARD – up to $1,500,000 for members (up to $500,000 for eligible spouses) under age 65. Apply using our standard form (health questionnaire and, in some cases a paramedical exam and blood test may be required at no cost to you).
