- Our "request for quote form" requires
the following personally identifiable
information : name, gender, age, address, marital and family status.
Additionally, we ask for information relating to tobacco use and any current health
insurance arrangements.
Our full application for health insurance
requires detailed health information on all applicants for insurance. In addition to the
personally identifiable information noted above a full medical history, including names
of attending physicians, illnesses and medications used is required.
However, this full health insurance application
is only available to be printed from our site. It cannot be submitted electronically and
will not be accepted as an Email or attachment to an Email. The form should be completed
on line and then printed (alternatively it can be printed and filled out by hand),
signed and sent to Affinity Group Underwriters by fax or regular mail.

- The information is used initially to create a
personal health insurance "quote" for you and/or your family.
Subsequently, if the quote is accepted by you and a detailed Health Insurance
Application is submitted, the additional information is used to "underwrite"
(determine eligibility for a Medical Insurance policy) for you and/or your family.
Personally identifiable information such as name,
address, or Social Security number is collected as part of the process of requesting a
quote. Companies use your personally identifiable information in combination with other
information you provide about your insurance needs. Where permitted by law, your
personally identifiable information may also be used to request a credit report or your
claims history from a third party, or to verify other information you have provided.

- The information may be shared with our insurance
company partners, the Medical Information Bureau (MIB) and/or other authorized third
parties (e.g. your doctor).
There is a disclosure statement on all of our
Health Insurance Applications that advises of the dissemination of information and
further, it requires your signature as authorization to release the information.

- The choices
available to you regarding collection, use and distribution of the
information are limited as collection of the information and distribution of it to
relevant parties is a necessary part of the underwriting process.
However, if you reside in an area where none of
our insurance company partners have a health insurance product available, Affinity
Group Underwriters will obtain your authorization before releasing any information
to other insurance companies.

- The security procedures that
are in place to protect the loss, misuse or alteration of information under Affinity
Group Underwriters control include but are not limited to the use of the SSL (Secure
Sockets Layer) protocol for our on-line "quote request form".
With SSL, when you enter information into our
"quote request form" your information is encrypted as it travels between your
computer and our servers. Encryption makes it exceedingly difficult for others to view
your information.
The detailed medical information that you
provide in a "Health Insurance Application" is transmitted to our underwriting
partner(s) by secure fax or courier.

- You can correct inaccuracies in the
information by clicking the "Reset" button on the "request a
quote form" or the on-line version of the "Health Insurance
Application".
If you notice an error after you have sent a
form to us (electronically or by fax or mail), you may send an Email to EParrrish@agu.net
or call us (toll free) at 1-877-673-9797 and we will make the necessary corrections.
You can change the information you provide at any time. Your right to review and
correct information obtained from third parties, such as credit reports, is subject to a
variety of federal and state laws.

- No, Affinity Group Underwriters uses your personal information only for
the preparation of insurance quotes and insurance policies. Unless you expressly give us
permission to do so, or unless required by law, we do not willfully use or disclose
personally identifiable information.
However, we may share aggregated statistical
information from our database with our participating insurance companies or business
partners. Even in this situation, individual applicants' personally identifiable
information is not disclosed.

"Health Insurance Application", one of our Customer Service Representatives

system, internet browser and e-mail client (program for accessing emails) that you are
using.
This allows us to notify you of potential
problems with our applications and certain operating system-internet browser
combinations. Determining the e-mail client in use allows us to send communications that
are compatible with your system.
